Payments

If you have UK-based medical insurance (BUPA, AXA, Vitality, AVIVA, WPA, CIGNA, Helix, Allianz) please provide your membership or plan number as well as the authorisation code for the appointment.

If treatment is being sponsored by an Embassy, etc, please ensure that a Letter of Guarantee (LOG) is provided prior to the appointment.

Please be aware that it is your responsibility to ensure that Mr Hartley’s invoices are settled in a timely matter – if there is a shortfall of payment or non-payment from your insurance company/Embassy you will be asked to pay the outstanding balance.

In the event of long-standing non-payment Mr Hartley reserves the right to pass your details to a debt collection agency to recover any outstanding funds. You will then be responsible for any fees incurred by this agency.
