Payments

If you have UK-based medical insurance (BUPA, AXA, Vitality, AVIVA, WPA, CIGNA, Healix, Allianz) please provide your membership or plan number as well as the authorisation code for the appointment. Some insurance companies, i.e. Healix provide Guarantee of Payments for your child’s consultation/treatment.

If treatment is being sponsored by an Embassy, etc, please ensure that a Letter of Guarantee (LOG) is provided prior to the appointment.

Please be aware that it is your responsibility to ensure that Mr Hartley’s invoices are settled in a timely matter – if there is a shortfall of payment or non-payment from your insurance company/Embassy you will be asked to pay the outstanding balance.

Any overdue payments/debts will be pursued through the HM Courts & Tribunals Service. You will then be responsible for any fees incurred by this agency.
