Payments

If you have UK-based medical insurance (BUPA, AXA/PPP, Vitality, AVIVA, WPA, CIGNA, Allianz) please provide your membership or plan number as well as the authorisation code for the appointment.

If treatment is being sponsored by an Embassy, etc, please ensure that a Letter of Guarantee (LOG) is provided prior to the appointment.

Please be aware that it is your responsibility to ensure that Mr Hartley’s invoices are settled in a timely matter – if there is a shortfall of payment or non-payment from your insurance company/Embassy you will be asked to pay the outstanding balance.

In the event of long-standing non-payment Mr Hartley reserves the right to pass your details to a debt collection agency to recover any outstanding funds. You will then be responsible for any fees incurred by this agency.
